If you are a dermatologist, surgeon or other health care provider experienced in treating people with hidradenitis suppurativa, or are actively involved in researching the disease, you should be listed in the HSF Physician Action Network. Your free listing will provide general information for patients; other medical professionals helping patients locate appropriate services will also find this resource invaluable.

Completing the form should only take a few minutes: You can either print it out and write the information in manually, OR type in the information, print it out, and sign it.
